MTM Program Completion Rate for CMR

A Part D measure that reports the percentage of Medication Therapy Management (MTM) program enrollees who received a Comprehensive Medication Review (CMR) during the reporting period. ElementDefinition
Eligibility (CY2025 criteria)Multiple chronic diseases from the 10 core chronic diseases, which now include HIV/AIDS; multiple Part D drugs; and an annual Part D cost threshold tied to the average cost of eight generic drugs. Members in a drug management program are also eligible. Enrollment is opt-out.
DenominatorMTM enrollees enrolled in the program for at least 60 days during the reporting period, plus enrollees with fewer than 60 days who received a CMR in that time.
NumeratorEnrollees who received at least one CMR during the reporting period.
CMR standardAn interactive, real-time, person-to-person review, in person or by telehealth, with the member or an authorized representative, followed by a written summary in the CMS Standardized Format.
Data validationContracts scoring below 95% on MTM data validation do not receive a rate.

The MY2027 specification will be published in the 2029 Star Ratings Technical Notes. Because the returning measure is treated as new, confirm denominator rules against that release.

From eligibility to a counted CMR

Step 1

Eligible

Meets the plan's targeting criteria under the 2025 rules.

Step 2

Enrolled

Opt-out enrollment places the member in the denominator once timing rules are met.

Step 3

Engaged

Member or authorized representative is reached and agrees to the review.

Step 4

Counted

CMR completed and written summary delivered in the Standardized Format.

Figure 1. The denominator is set at enrollment; only completed, documented CMRs reach the numerator.

Clinical and operational considerations

  • A CMR is a clinical encounter. It should reconcile medications across prescribers, assess each drug for indication, effectiveness, safety, and adherence, and produce a medication action list the member can use.
  • Authorized representatives count. Cognitive impairment is common in MTM-eligible populations, and a caregiver can complete the CMR on the member's behalf.
  • The 2025 eligibility expansion grew the denominator. Capacity planning for MY2027 should start from the expanded eligible population, not historical volumes.
  • Documentation is part of the measure. A review without a compliant written summary does not count, and data validation failures remove the rate entirely.
  • Cut points reset. As a new measure, there is no prior-year cut point and no guardrail limiting movement in its first year.
  • Language access affects reach. Outreach in the member's preferred language improves contact and completion in diverse populations.
